List of documents to be submitted along with the form
1. A passport size photograph of the student and one of each parent.
2.A photocopy of a birth certificate issued by the Municipal Corporation/Civic Authorities.
3.A Photocopy of the latest report card.
4.A Photocopy of the latest report card.
5.A Photocopy of the Residential Proof (copy of the Ration Card/Voter’s ID/Passport].
6.A Photocopy of the passport if the student is a non-resident Indian (NRI).
